In the heart of San Francisco’s Union Square, Delugan Meissl Associated Architects (DMAA) has created a surgeon’s office unlike any place you visit in medical school. The Deschamps-Braly Clinic of Plastic & Craniofacial Surgery is no ordinary medical practice, either. Dr. Jordan Deschamps-Braly is world-renowned for, as he describes it, “gender-affirming procedures for transgender individuals who need their face feminized or masculinized,” which accounts for over 50% of his work.

“Ever since high school, I’ve been saving design clippings,” Deschamps-Braly says. “At some point I ran across an article about a penthouse residence called Ray 1, which basically looks like a spaceship landed on top of a six-story building in Vienna.” The project was designed by Austrian firm Delugan Meissl Associated Architects: “I emailed their office not expecting to get an answer, thinking that this project was way too small for them.” Then he did. For more on how the sleek offices came together, AD PRO spoke with Diogo Teixeira, DMAA’s project manager.

AD PRO: What was the existing physical space like? What did your team have to work with?

Diogo Teixeira: We basically had an empty suite to bring our client’s dream into reality. However, the space’s L shape left a considerable area without sunlight. For us, it was very important [to determine] how the light could come inside the space.

AD PRO: Did you leave any of the elements untouched?

DT: Essentially, we undressed the entire suite to its raw state. Even the exterior window bars—they interfered with our idea that the windows would be frames for the exterior urban picture.

Wood lamellae run across the floor, ceilings, and walls. All were fabricated remotely in Vienna before being assembled on-site.

AD PRO: This was your first time designing for an operation like this one. For this type of project, where privacy is important, what were some of the design tricks you used?

DT: We have experience in health care facilities, but this is certainly a unique one. Privacy is definitely one of the keys here, which we achieved not only in the spatial organization, but also how we framed the office with the timber elements. Locating the exam rooms and waiting area in the back of the space, away from the public eye, was intentional to give the patients privacy. Another vital premise for us was to demystify the sterile and dry connotations such practices normally have. So, the spatial atmosphere is calmer and warmer than usual. It really matters how the patients perceive and interact with the space.
